Patient Information

We are contracted with the following insurances:

  • Medicare
  • ODS (Moda)

There are two types of coverage for eye care.  Vision care insurance is used for routine eye exams, glasses and contact lenses.  Medical insurance is used for medical eye problems, such as diabetes, glaucoma, macular degeneration, flashes, floaters, eye pain, dry eyes, red eyes, etc.

If you have both types of insurance it may be necessary for us to bill some services to one plan and other services to the other.  We will use coordination of benefits to do this properly and to minimize your out-of-pocket expense.

Forms of payment may include:

  • Cash
  • Check
  • Money order
  • MasterCard
  • Visa
  • American Express
  • Discover
  • CareCredit*

*CareCredit is a medical credit card, which may also be used in our optical department for purchases over $200.  Our staff will be happy to assist you should you be interested in hearing more about this payment option.
